Yes, a valid referral needs to be sent to North Coast Rheumatology. Dr Ward reviews each referral. You or your referrer may be contacted if further information is needed to assist in triaging appointments.

You will be sent a Health Assessment Form to be completed prior to your first appointment. Please allow 30 to 60 minutes for completion of this form. This form will include information regarding a current medication list. Please phone the practice on 02 5631 1756 if you have any troubles completing this form.

Please check prior to your appointment if you are due for repeat prescriptions of current rheumatological medications. This may require checking with your pharmacist.

If you have any symptoms of COVID-19, please do not attend the appointment. Please phone the practice on 02 5631 1756 as early as possible to allow rescheduling of appointment.

Please bring a current list of all medications (including any inhalers, creams, injections, monthly/yearly medications) to every appointment.
